Integrated Behavioral Health

Access to behavioral health care remains a significant challenge across the United States. In fact, nearly 50% of all behavioral health disorders are treated exclusively by primary care physicians. The Collaborative Care Model addresses this gap by integrating behavioral health services into everyday medical care, patients receive timely, comprehensive treatment with the comfort and familiarity of their providers, reducing barriers to access and improving outcomes. CoCM is first and foremost a telehealth-based program that allows patients to receive virtual consultations, making care more accessible and efficient. It addresses a wide range of behavioral health issues—most commonly depression, anxiety, and substance or alcohol use disorders—directly within the primary care setting. CoCM enables primary care providers to identify high-risk patients early and coordinate preventative care through a collaborative, team-based approach.

How It Works

Instead of months-long waits to see a behavioral health specialist, patients can receive mental health care quickly and conveniently—often within days—through CoCM. The primary care provider identifies patients who may benefit from support, and a behavioral health team steps in to offer coordinated, evidence-based treatment. Care is delivered through ongoing virtual or in-person touchpoints, making it easier for patients to stay engaged in their treatment.
