Platinum Healthcare is a care management agency for New York State’s Health Home program through Medicaid with the goal of improving quality of life for a high-needs population. Health Home is a care coordination program for patients with a minimum of two chronic medical and mental health conditions. Our Health Home program is designed to ensure members receive the care, support and services they need to stay healthy and prevent hospitalizations. We work closely with individuals during at home visits helping members understand their treatment, set and reach health goals, and advocate for them during medical appointments when needed. We empower our patients to take control of their well-being, build confidence, and enjoy a better quality of life. Through the Health Home program, members can access help connecting to medical, mental health, and substance use treatment, along with support in applying for benefits like SSI and SNAP, scheduling appointments, and transportation assistance. Platinum is currently providing care coordination for Health Home in Manhattan and The Bronx.
